* Birth document (certificate, hospital copy, etc.)
* Current Immunization Records
* Proof of Residency in Mabton (physical address)
In an effort to maintain a healthy environment, we have updated our registration process.
1. We ask that you please call to schedule an appointment to register your student. (30-minute appointments) Call (509) 894-4941.
2. Unless necessary, please do not bring others to your appointment.
3. If you are experiencing or exhibiting any symptoms of being sick or have been exposed or had close contact with a person who has been diagnosed with a cold or virus, please stay home. You can call to reschedule your appointment for a later date.
If you are unable to come in for an appointment, you can call to request a packet to be completed at home.
**Packets that are not completed on site will be processed at a later date. We thank you for your patience & assistance with this process.**
